Attend Worcester youth resource and employment related meetings, as appropriate.       <title>Family Support Advocate</title>
      <description><![CDATA[<p>Through our Healthy Families, Early Head Start and Head Start Programs, Worcester Community Action Council (WCAC) provides a continuum of early education and care programming for families&nbsp;throughout&nbsp;Southern Worcester County&nbsp;prenatally through Kindergarten enrollment. We strive to support positive parent/child interactions through developmentally appropriate curriculum and activities for the whole family to participate in.&nbsp;Healthy Families is a home-based family support and coaching program that supports young, first-time parents and helps them create stable, nurturing environments for their children.&nbsp;<br></p><p>WCAC is seeking a full time Family Support Advocate to&nbsp;partner with first-time, young moms and dads to provide coaching and support navigating the ups and downs of parenting. The Family Support Advocate will provide program participants with home visits, advocacy, support, assistance, and education to help them achieve optimal health, development, family stability, and function in their communities in accordance with a participant generated family goal plan.</p><p>WCAC offers a competitive salary and excellent benefit package including health, dental, vision, life, 403(b) retirement plan with Agency contribution, flexible spending account, paid holidays, vacation, personal and sick time.</p><p><u><span>Qualifications:</span></u></p><p></p><ul><li>Minimum High School Diploma or HiSET equivalency. Associateâ€™s degree preferred.&nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</li><li>Experience working with infants, toddlers, adolescents, and young families.</li><li>Experience working with adolescents in one-on-one and group settings.</li><li>Experience working with diverse populations.</li><li>Ability to demonstrate empathy and advocacy for participants.</li><li>Demonstrated ability to establish supportive, non-judgmental relationships with others.</li><li>Expressed desire and motivation to provide home visiting services to infants, toddlers, adolescents, and families.</li><li>Excellent verbal and written communication skills.</li><li>Ability to speak Spanish, a plus.</li></ul><p><span><u>Duties &amp; Responsibilities:</u></span></p><ul><li>Maintain caseload up to twenty participants.</li><li>Work with each participant family to coordinate services, report observations, and seek instruction and guidance specific to each family's needs.</li><li>Educate participant families about the importance of early and continuous prenatal care and positive habits to enhance pregnancy outcome.&nbsp;</li><li>Educate families on infant, toddler and adolescent development, family health and nutrition as well as self-sufficiency and safety.</li><li>Assist parents and family members in learning effective interaction and parenting skills with infants and toddlers through positive role modeling and providing current information on relevant topics for the parents.</li><li>Support young parents in their social, environmental and health needs by identifying services, community resources and informal networks.</li><li>Assist participant families to identify their individualized goals and objectives and how to plan for successful achievement of their plan. Help them to identify barriers and solutions to overcome the barriers through self- sufficiency or available services or programs.</li><li>Assist participant families to learn parental coping strategies, and to expect realistic accomplishments of their child based on their childâ€™s developmental stage.</li><li>Make the necessary referrals to appropriate services, with participantsâ€™ consent, such as Early Intervention, when ASQ scores are low.</li><li>Assist with preparation, facilitation, and co-facilitation of group sessions. Provide child care for participant group sessions as assigned.</li><li>Coordinate transportation for participants to attend group programs and events if pre-approved.</li><li>Participate in case conferences and coordination, bi-weekly team meetings, and individual weekly supervision.</li><li>Identify new referral sources and recruit new participants for program through community outreach and collaboration with other human service providers.</li><li>Provide referral assistance for families contacting the program who do not meet the criteria for Healthy Families of Southern Worcester County. We currently operate two Head Start Centers, one in Southbridge operating&nbsp;8&nbsp;classrooms and the other in Webster with 4 classrooms.&nbsp;Both centers are licensed through EEC, accredited through NAEYC, and QRIS rated level 3.&nbsp;Head Start prepares pre-school aged children for school by offering&nbsp;center-based&nbsp;safe and&nbsp;fun&nbsp;education programs which incorporates learning and social/emotional skill development.â€¯ Children learn the â€œKindergarten readinessâ€ skills they need to do well in school.â€¯ Families receive the support they need to meet and advocate for the needs of their children.&nbsp;&nbsp;<br></p><p>We are seeking a Family Advocate to join our team.&nbsp;<span>The Family Advocate is responsible for working with families to help them realize their self-sufficiency goals. The successful candidate will support the role of parents and family well-being by encouraging parent involvement in Head Start while providing advocacy, case management and social service guidance based on family goals. Responsible for recruiting eligible children and introducing families to all Head Start options and other WCAC programs. Identifies child and family needs for center-based families and provides referrals, advocacy, and follow-up.</span>&nbsp; This is a full time, school year position,&nbsp; Monday to Friday 8:00am - 4:00pm based at our Southbridge center. Bilingual candidates are strongly encouraged to apply.</p><p>WCAC offers a competitive salary and excellent benefit package including health, dental, vision, life, 403(b) retirement plan with Agency contribution, flexible spending account, paid holidays, vacation, personal and sick time.</p><p><span><u>Qualifications:</u></span></p><ul><li>Associate degree in Human Services and at least 2 yearsâ€™ experience in human service field.</li><li>Ability to work cooperatively and effectively with a diverse population.</li><li>Knowledge of community resources in Southern Worcester County.</li><li>Strong computer skills and working knowledge of Microsoft Office and the ability to learn agency systems such as ChildPlus.</li><li>Excellent interpersonal and communications skills, verbally, written, and virtual.</li><li>Bilingual preferred.</li></ul><p><u><span>Duties and Responsibilities:</span></u></p><ul><li>Assist with the recruitment and intake process of new families.</li><li>Ensure weekly meetings are completed to discuss 3-5 children and communicate with supervisor by email when meetings do not occur and the reason. Provide documentation of weekly meetings.</li><li>Coordinate and lead Case Management meetings and be prepared to report outcomes on any referrals, follow-up and actions discussed. Utilize case conferences with outside community agencies as necessary.</li><li>Conduct a minimum of two (2) home visits per year, per family, (including summer months) or as needed, to facilitate family plans, assist families in identifying and achieving needs, follow up on referrals and to work on the Family Partnership Agreement.</li><li>Contact all families monthly to serve as a â€œcheck-inâ€. When a family falls into a high-risk category, Family Advocate must conduct a home visit at least once a month.</li><li>Schedule and attend monthly workshops, speakers, and parent activities for the Center Committee meetings.</li><li>Observe in the classroom a minimum of 2 hours per month.</li><li>Meet with Early Head Start Staff monthly (at a minimum) to coordinate services for families in the dual program.</li><li>Provide a month end closing report to Family Engagement Coordinator.</li><li>Monitor attendance of enrolled children and conduct a home visit when a child has been absent for two consecutive days without any contact. </li><li>Coordinate orientation process for families with the site supervisor. Prepare orientation paperwork and attend sessions.&nbsp; &nbsp;</li><li>Complete all required documentation pertaining to the program using electronic database management system (Child Plus). Ensure that all related family information is entered, scanned, and updated in a timely manner; Including but not limited to communication with parents, PIR information, upload necessary documentation that include family reviews, case management, custody documentation, and any other documentation related to Social Service. Assure documents in childrenâ€™s files match ChildPlus data.</li><li>Develop a working knowledge of community resources and act as liaison between community resources and families. Make referrals and arrange transportation with families when necessary to community service providers, medical, and dental offices, and other appointments as necessary.
